Job Description Job Description PURPOSE OF POSITION Covenant
House Missouri (CHMO) welcomes youth who are runaway, trafficked,
at-risk, or experiencing homelessness with unconditional love,
absolute respect and a place to call home. Beyond shelter and basic
needs, we deliver transformative solutions to help young people
ages 14 to 17 build productive, independent and sustainable lives.
The incumbent is responsible for providing goal-oriented support
and individualized direct care and supervision of program youth.
This position is focused on achieving individualized goals for the
persons served by assessment, planning, advocacy, and coordination
in a manner which reflects the mission, philosophy and policies of
Covenant House Missouri. SUMMARY OF DUTIES (Responsibilities to
include but not limited to the following) Establish and maintain a
good working relationship with law enforcement and the office of
violence prevention Responsible for conducting a comprehensive
assessment for all youth referred. Identify the youths need and
connect the youth and their families to relevant services
providers. Record programmatic activities using efforts to outcomes
(ETO) software system. Make referrals and follow up with clients
maintaining ongoing engagement. Track and report outcome data on
youth and families at 3, 6, 9, and 12 month post-initial
assessment. Provide a programmatic monthly report to funded source.
Accept referrals from police officers BHR, OVP and all other social
service providers as appropriate. Recognize the importance of
community integration in reducing youth homelessness and engagement
with law enforcement. Initiates crisis intervention counseling to
youth and to families as needed. Support a trauma Informed culture
through treatment planning and daily interactions, advocating with
and on behalf of individuals, according to their needs and wishes.
Youth in the community aftercare-based Programs Provide information
or refer individuals to established and preferred public or private
agencies or community services for assistance. Responsible for
establishing and maintaining good relationships with appropriate
boundaries with youth. Assess and meet youth's immediate basic
needs such as food, clothing, medical attention etc Manage the
day-to-day workflow while providing direct care, supervision, and
programming for youth, in accordance to CHMO policies and
procedure. Transport and accompany youth home, to appointments and
partnering agencies in company vehicles. Ensure that the facility
is secure and well-maintained at all times. Report all maintenance
concerns to the onsite manager in a timely fashion. Maintain an
environment in which the needs of the youth are addressed in a
manner that is comfortable, motivating, and consist with CHMO
service philosophes. Conduct Life skills training and safety
related drills to the youth. Provide agency presentations in the
community to schools, service providers, and other community based
groups and organizations. Represent the agency at community based
fairs and events. Lead, plan and/or participate in group activities
and meetings for youth as assigned. Record and maintain records,
required paperwork, and documentation according to agency policy
and procedures. Assist the onsite manager in planning appropriate
community-based and leisure activities for youth. Participate in
all required trainings and staff meetings, completing at least 40
hours of training annually. Support CHMO service
Philosophies-Housing First, Harm reduction, Trauma Informed Care,
Strength Based approach, and positive Youth Development- including
acting in accordance with all organizational and legal protocol,
policies, and procedures. Participate in the organization Quality
Improvement Program through incident reporting and identifying
opportunities for improvement. Ensure confidentiality of youth
personal information. Perform other duties as assigned. SUPERVISORY
DUTIES No direct reports WORKING RELATIONSHIPS Regular internal
contact with all CHMO staff, outside contact with vendors and
suppliers, and regular contact with outside agencies. KNOWLEDGE,
SKILLS, & ABILITIES Education & Experience Bachelor's degree
required Two (2) years' experience preferred. Driver's License
required Knowledge/Skills The ability to work cooperatively with

pertaining to the delivery of human services. Fundamental knowledge
in mental health, trauma, substance abuse, homelessness, etc.
Proficient in Microsoft Office, assessment tools, database systems,
and Windows environment. WORK ENVIRONMENT/ PHYSICAL DEMAND Moderate
concentration. Frequent interruptions. Moderate noise level.
Moderate sitting, standing and eye strain, light walking, bending
