Job Description:

will play a pivotal role in our integrated health care delivery
system. This position is key to enhancing patient care by guiding
patients through complex health care processes, coordinating care
plans, and ensuring a holistic approach to health and wellness. You
will collaborate with multidisciplinary teams to provide tailored
healthcare services that align with our mission of compassionate
service. Assess patient needs and develop individual care plans in
collaboration with other healthcare professionals. Act as a liaison
between patients, family members, and various healthcare
specialists. Navigate patients through the healthcare system,
ensuring they receive timely and appropriate care. Educate patients
and their families about their conditions and the necessary steps
for recovery. Monitor patient progress and adjust care plans as
needed. Document patient interactions, treatments, and outcomes for
continuous improvement in care delivery. Strong understanding of
healthcare systems and policies. Excellent communication and
interpersonal skills. Ability to handle sensitive information
confidentially. Strong organizational and problem-solving skills.
Proficient in electronic health record (EHR) software. Medical
Insurance Dental Insurance Vision Insurance Life Insurance 401k
Valid registered nurse (RN) license in the state of practice.
Certification in Care Coordination and Transition Management
(CCCTM) preferred.
