The Home Health and Hospice Intake Nurse (RN), reporting directly
to the Executive Director: Serves as the clinical gateway to care,
ensuring patients experience a seamless, compassionate transition
into home health or hospice services. Evaluates referrals, assesses
clinical eligibility, and coordinates timely initiation of services
in collaboration with physicians, hospitals, and interdisciplinary
team members. Acts as a trusted resource for patients, families,
and referral partners, balancing clinical judgment, regulatory
compliance, and exceptional customer service to support
high-quality, patient-centered care. Essential Duties: The
following duties are normal for this position. This list is not

Reasonable accommodations may be made to enable individuals with

and clinically evaluate referrals from hospitals, physicians,
skilled nursing facilities, and community partners. Determine
appropriateness for home health or hospice services based on
diagnosis, payer source, and regulatory criteria. Collect and
verify required clinical documentation, demographics, insurance
information, and physician orders. Collaborate with physicians,
case managers, and internal interdisciplinary teams to develop
individualized plans of care. Schedule timely start-of-care visits
and assign appropriate clinical staff based on patient needs and
acuity. Communicate admission details and special considerations to
field clinicians to ensure continuity of care. Accurately enter and
maintain patient records within the electronic health record in
accordance with agency policy, state/federal regulations, and HIPAA
standards. Verify payer eligibility and authorization requirements;
coordinate with billing as needed. Ensure all consents, orders, and
admission documents are complete prior to initiation of services.
Educate patients and families regarding available home health or
hospice services, visit frequency, and expectations of care. Other
duties as assigned. Qualifications, Education, and/or Experience:
Current and unencumbered Registered Nurse (RN) license in the state
of Missouri. Minimum of 2 years of clinical nursing experience,
preferably in home health or hospice settings. Strong
organizational skills with the ability to manage multiple tasks
simultaneously. Excellent communication and interpersonal skills.
Proficiency in electronic health records (EHR) systems and basic
computer applications. Experience in patient intake or case
management. Knowledge of Medicare and private insurance regulations
related to home health or hospice services. Familiarity with local
healthcare providers and referral sources in the St. Louis area.

Conditions: This position may involve a range of physical
activities, including those outlined in the Essential Duties, but
is not limited to them. Frequent travel within assigned service
area is required. While performing duties, the employee may
occasionally be required to stand for extended periods, move,
communicate, and identify written information. Occasionally
required to climb or balance, stoop, kneel, crouch, or crawl;
lift/move up to 50 pounds independently, and up to 200 pounds with
assistance. Work is performed in patient homes, care
settings/facilities, and office settings, navigating a variety of
environments to provide care. Exposure to varied home environments
and potential infectious conditions. Must comply with all Dover
Health health and safety requirements. Actively participate in
