Job SummaryMercy Home Health is seeking a skilled and compassionate RN to provide high-quality nursing care to patients in their homes. As a Home Health Nurse, you will assess, plan, implement, and evaluate patient care, working collaboratively with the healthcare team to ensure optimal outcomes.Responsibilities

• Provide skilled nursing services to patients in their homes, including assessment, planning, implementation, and evaluation of care
• Develop and implement individualized care plans to meet the unique needs of each patient
• Collaborate with the healthcare team to ensure seamless care coordination and communication
• Conduct thorough assessments of patients' physical, emotional, and social needs
• Develop and implement strategies to promote patient safety, independence, and well-being
• Document patient progress and outcomes in a timely and accurate mannerRequirements
  • Current RN license in the state of Missouri
  • One year of experience in a related field
  • Graduation from an accredited nursing school
