Overview Position Title: Clinical Consultant- SNF Clinical
Quality and Support Reports To: Director of Clinical Quality and
Development FLSA Status: Exempt Direct Reports: Yes Department:
EmpowerMe Rehabilitation Position Summary: As the Clinical
Consultant, reporting directly to the Director of Clinical Quality
and Development, you will be responsible for the support of newly
onboarded therapists and clinical staff on organization-wide
initiatives and quality assurance programs in accordance with
Federal, State and Local regulations. This role also supports each
team in optimizing each resident---s well-being through clinical
education, regulatory compliance, and clinical practices within
each location. Responsibilities Provides support in the
implementation of clinical programs and evidenced based practices
that meet the needs of patient and residents in a variety of
settings. Assists operations leadershipith clinical training and
ongoing support of newly hired clinicians at new clinic sites,
existing clinical sites, and transitional clinic sites. Works to
provide staff with the necessary resources and guidance to make
orientation process as smooth and efficient as possible. Provides
feedback and education to each clinician following completed
clinical record reviews on quality and compliance of documentation
and billing practices Monitors Clinical Support help desk and
provides education in alignment with best practices and the
organization---s mission. Identifies the unique needs at new,
existing, and transitioning clinic sites and provides necessary
support to meet these needs. Assists in the implementation and
monitoring of policies and procedures within each community,
including during acquisitions and the opening of new communities
Demonstrates competence in regulatory compliance and provides
regular education to newly hired clinical staff and operations
leadership so that that each community maintains compliance with
all regulatory agencies Regularly reviews, audits and conducts
analysis of clinical services data to identify areas for
opportunity and adherence to current clinical policies and
procedures. Creates, refines, and implements clinical pathways to
improve clinical quality Provides feedback and education to
operations team based on clinical services data and value-based
outcome measures. Assists in the continued education on new and
updated policies and procedures in Patient Care Manual, Regulatory
Manual, Emergency Preparedness manual, as well as corresponding
policies and procedures that are in accordance with Federal, State
and Local regulations Collaborates with operational and clinical
team members to assist in implementation of strategic initiatives.
Networks and creates relationships within the assisted living and
long-term care industry Aligns his/herself with owners--- vision
for service quality and value creation Other duties as assigned
Qualifications Five (5) years of experience as a Speech Language
Pathologist in a Skilled Nursing Facility with proven leadership
and organizational skills Great analytical, communication and
interpersonal skills are required along with the heart and passion
to work with older adults while providing the best care possible
Proven experience in training, developing and leading strong
therapy teams Must have the ability to remain calm in stressful
situations, be flexible, work well with many interruptions, and to
effectively balance multiple projects at one time Respect for the
principles of resident rights, including privacy and
confidentiality Education and/or Experience: A minimum of Five (5)
years of experience as a Speech Language Pathologist Applicable
State licensure(s) Other Qualifications: Must possess strong
knowledge of laws and restrictions regarding Physical,
Occupational, and Speech Therapy practice, including applicable
state practice acts Knowledge of federal, state, and local
regulations as they pertain to long-term care, skilled nursing, and
rehabilitation facilities Must exhibit and promote a high level of
professionalism, customer service, curiosity, and friendliness in
all interactions with employees, residents, and visitors throughout
the organization Ability to travel overnight as needed with
occasional excessive travel Computer Skills: Proficiency in
Microsoft Office applications, Electronic Health Records
applications, and Internet browsers. Physical Demands: The physical
