Hospital is seeking an RN Navigator. This role will be responsible
for running Code Stroke alerts in house and in the Emergency
department. We prefer experience in Neuro and ER for this role. No
weekends or holidays are required. Job Summary:Provides services as
a patient navigator throughout the continuum of care. Job
Responsibilities and Requirements:PRIMARY
RESPONSIBILITIES\nFunctions as a liaison for management staff,
administration, physicians, managed care companies, community
organizations, and other customers.\nCollaborates with physicians
and advanced practice providers to ensure quality outcomes.
Coordinates efforts for treatment and promotes quality improvement
and educational efforts.\nCompiles data, tracks outcomes, and makes
recommendations for process improvement.\nProvides
referrals/support for follow-up care for patients who are uninsured
or underinsured.\nUtilizes the nursing process of assessment,
planning, intervention, and evaluation in all patient
encounters.\nAssists medical provider with processing of diagnostic
results bringing significant values to the provider's attention.
Exhibits knowledge of tests and procedures and their significance
as related to patient care.\nRecognizes the implementation of
dosages, interactions, side effects, adverse effects, routes of
administration of drugs as they apply to individual patients. Uses
knowledge of medications in instructing patients about medication
therapy.\nEstablishes working relationships teaching protocols for
patient education, family counseling and general public
information.\nWorks in a constant state of alertness and safe
manner.\nPerforms other duties as assigned.\nEDUCATION\nGraduate of
accredited school of nursing or education equivalency for
licensing\nEXPERIENCE\nThree years' registered nurse
experience\n\nPHYSICAL REQUIREMENTS\nConstant use of speech to
share information through oral communication.\nConstant standing
and walking.\nFrequent lifting/carrying and pushing/pulling objects
weighing 0-25 lbs.\nFrequent sitting, reaching and keyboard
use/data entry.\nFrequent use of vision and depth perception for
distances near (20 inches or less) and far (20 feet or more) and to
identify and distinguish colors.\nFrequent use of smell to
detect/recognize odors.\nFrequent use of hearing to receive oral
communication, distinguish body sounds and/or hear alarms,
malfunctioning machinery, etc.\nOccasional lifting/carrying and
pushing/pulling objects weighing 25-50 lbs.\nOccasional
lifting/moving of patients.\nOccasional bending, stooping,
kneeling, squatting, twisting, gripping and repetitive foot/leg and
hand/arm movements.\nOccasional driving.\nRare crawling and
