Job Description:
GENERAL PURPOSEPerforms evaluations and develops effective
patient treatment plans to restore, maintain or prevent decline of
patient function, by planning and administering medically
prescribed therapy treatments in accordance with federal, state and
professional standards governing outpatient and home health care.
Services may be performed in therapy clinic or individual patient
residence.ESSENTIAL JOB DUTIESProvides initial physical therapy
assessment when requested in accordance with the patients plan of
care. Plans and administers prescribed physical therapy treatment
programs. Meets the patients goals and needs and provides quality
care by assessing and interpreting evaluations and test results;
determining treatment plans in consultation with physicians, and by
prescription. Helps patients accomplish treatment plan and secures
necessary supplies and adaptive equipment to facilitate
independence in patientsAdministers therapy treatments according to
a treatment plan approved by the attending physician. Directs
treatments given by assistants. Complies with regulatory conditions
of participation for outpatient and home health servicesEvaluates
effects of therapy treatments by observing, noting and evaluating
patients progress; providing medically necessary treatment plan and
modificationsDocuments patient care services by charting in patient
and department records according to accepted regulatory, corporate
and professional guidelines. Records daily treatment charges per
corporate procedures to allow accurate and complete billing.
Documentation may be completed using a company furnished electronic
device).Protects patients and employees by adhering to treatment
location infection control policies and protocols and keeping
information confidential per company and federal requirements.
Maintains safe and clean working environment by complying with
treatment location and department procedures, rules and
regulations.Provides services in multiple settings (including
outpatient clinics in stand-alone and host (i.e. AL, IL) locations
and patient homes) as determined by supervisor and patient/resident
needQUALIFICATIONSGraduate from Physical Therapy programCurrent
license as Physical Therapist in the state of practicePrevious
experience in outpatient, preferredMust meet Clinical Competency
requirementsAbility to travel within 50 minutes of your assigned
primary building(s) as needed.Current CPR certification, if
practicing in home healthMust be capable of maintaining regular
attendanceKNOWLEDGE, SKILLS, ABILITIES & BEHAVIORSAbility to
communicate in English, both verbally and in writingGood
problem-solving skillsGood computer skills with ability to learn
company or customer systemsMust be able to maintain confidentiality
regarding patient, employee and company proprietary informationMust
have the ability to relate professionally and positively and work
cooperatively with patients, families, caregivers, and other
employees at all levelsPHYSICAL AND SENSORY REQUIREMENTS Medium
work involves lifting no more than 50 pounds at a time with
frequent lifting or carrying of objects weighing up to 25 pounds.
If someone can do medium work, we determine that he or she can also
do sedentary and light work.Hearing (corrected) adequate for
oral/aural communication with patients, staff, family, visitors,
etc.Vision (corrected) adequate for readingIntelligible speech and
normal language/cognitive skillsMust be able to push patients in
wheelchairsLifting patients, equipment or supplies will be required
up to 20 pounds frequently, up to 50 pounds occasionally and up to
100 pounds rarelySitting, standing, walking required throughout the
dayJob duties sometimes require climbing stairs, kneeling,
twisting, bending; on occasion crouching, crawling and reaching
overheadMust be able to transfer patients.Must be able to
demonstrate any appropriate exercise and activities to
patients/caregiversWork in a fast-paced clinical environmentWeekend
and holiday work may be requiredWork environment is primarily
indoors but occasionally outdoors.Several tasks and job duties
involve a risk of exposure to Blood borne Pathogens and other
