  Overview
  Role PurposeProvides Diabetes and Nutritional educational services and community resources for inpatients and outpatients. Maintains a case load of patients and promotes program development. Responsibilities
• Monitors patients response and modifies plan of care accordingly.
• Provides accurate documentation as required.
• Delivers direct patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.
• Develops and implements individual plans of care in accordance with the established guidelines and standards of nursing care.
• Educates patients and their families on how to manage their condition.
• BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job. Minimum Requirements Degree
• Nursing Diploma/Associate's - Nursing/Nutrition/related Experience
• Supervisor Experience
• No Experience Licenses & Certifications
• RN Preferred Requirements and Additional Job Information Degree
• Bachelor's Degree - Nursing Licenses & Certifications
• Certified Diabetes Educator (CDE)
• CDL
• Cert Diabetes Educator Professional - Non Clinical Skills
• Health Education - 1 year or more
