Job Description:

The case manager is responsible for the delivery of wraparound case management services for the youth and their families, when applicable, that are enrolled in a variety of programs. The case manager will assess the needs of the youth and family, facilitate how these needs will be addressed, create treatment plans, coordinate care, and gauge the progress of the youth and their family. This position drives evidence-informed approaches, leads change efforts in support of St. Vincent's continuous improvement of clinical services and programmatic efforts, while ensuring the safety to all residents while empowering them in achieving their goals.QUALIFICATIONS: Graduate from an accredited college or university with a bachelor's degree in human services, or closely related field. Must be adaptable, flexible, and a strong leader with excellent interpersonal relationship and communication skills, a strong understanding of change management, and superb administrative skills.The ideal candidate should have at least one (1) year of experience involving the delivery of case management services and a strong sense of how to establish and maintain engagement within the community as it relates to the delivery of mental health treatment and social services. Additionally, experience and knowledge of adolescent development, child welfare, trauma-informed clinical approaches, and working with highly vulnerable youth (homeless or at risk of homelessness, backgrounds of abuse/neglect) is preferred as is experience with supportive housing and outreach services. Must have an acceptable background check and no record of child abuse.ResponsibilitiesConduct assessments with youth to determine barriers, resource needs, and other factors related to case planning, acting as a broker for services and referring them to community resources when necessary.Communicates and coordinates case management services with the clinical and supportive housing teams.Research available transitional, educational, and associated resources that will meet the youth's needs and links them to appropriate resources.Cultivate a working knowledge and relationship with relevant community resources to create linkages that meet the needs of the youth.Must employ appropriate and professional boundaries with the youth.Teaches job preparation and independent living skills in both an individual and group setting.Effectively able to manage and meet several deadlines simultaneously.Maintain an attitude of professionalism, collegiality, and optimism with both staff and youth to promote the welfare and well-being of the youth.Provide strengths-based case management and service coordination services designed to assist the youth in obtaining permanency.Maintain accurate and timely documentation that meets legal, agency, and contractual requirements, complete with weekly and monthly reports.Transport and accompany the youth to meetings and appointments as available and appropriate.Conduct aftercare services for youth and families and outreach services when needed.Attend all staff meetings, in-services, supervisions, and case conferences as directed.Employ Motivational Interviewing skills and other evidence-based techniques with youth to achieve consensus on individualized goals.Effective and concise writing skills that convey the desired message.Provide on-going monitoring and reporting of client's progress.Coordinate services and resources for physical and behavioral health and wellness and secure LGBTQIA+ supports within the community as needed.Effective knowledge in the areas of mental health issues, homelessness, trauma, and substance use disorder.Coordinate and provide access to educational and vocational training and supports.Work effectively with all systems involved in the youth's life including juvenile justice, schools, and law enforcement.Must subscribe to a teamwork mentality and have the ability to build trust and confidence within the team framework.Must have the ability to compromise when conflicting opinions exist while remaining focused on shared goals and vision.The ability to establish effective relationships with all levels within the agency as well as with the general public and representatives of other agencies and organizations.Perform other related duties as assigned.
